How elevate works with you

Fast, clear OT support for adults across Perth. 

 

Step 1 - Send a referral (2 minutes) 

Choose NDIS, Support at Home, or private referral and complete our short online form.

 

Step 2 - We contact your client (within 1–2 business days)

Our designated admin team calls the client or family to book a home visit at a time that suits them.

 

Step 3 – In‑home assessment & 48‑hour report
We complete the assessment, provide practical recommendations, and send a clear report back to you.
